Costs & Financial Aid

You are important to us.

At CHC, it is very important to us that your family receives needed services. We want you to know we have financial scholarships that may be available to you to help with the cost of evaluations and therapies delivered by CHC. We encourage you to apply.

Sliding Scale for Clinical Services

To determine eligibility, CHC takes several things into consideration including annual household income, number of individuals supported by your family’s income, living expenses, clinical needs, insurance coverage, and special situations facing your family.

To apply, complete the Sliding Scale Application and return it to your Care Manager via email or fax (caremanagers@chconline.org, fax 650.688.3669). Feel free to contact your Care Manager if you have questions. 650.688.3625. Your information will be kept confidential.

Evaluation Follow Up at No Cost

Once your family has completed an evaluation, participated in the follow up and received a comprehensive report from us, you are invited to return to CHC at no additional cost if you have questions to get further guidance. Your Care Manager can provide details. Please call 650.688.3625.

Santa Clara Medi-Cal

We provide expert behavioral health services in both English and Spanish to those served by Santa Clara Medi-Cal, ensuring cost isn’t a barrier to getting help. We bill services directly to Santa Clara Medi-Cal, so there is no cost to the family for our services.

We Accept Insurance for the IOP

CHC is not currently an in-network provider. We are considered out of network by all insurance companies. However, we do bill insurance companies for the IOP. In order for this to happen, CHC needs to be on your insurance company’s list of out of network providers. We bill them through what is called a Single Case Agreement. Please understand that if we receive an insurance payment that is not for the IOP, we return it to the company and make sure the family is reimbursed.
