Care Coordinators are the first point of contact for all new families interested in coming to CHC. When you call for help, a Care Coordinator will listen to your concerns and answer any questions you may have. If you would like to move forward with services they will match you with the best specialist for your child and set up your first appointment, or refer you elsewhere.
When you arrive for your first appointment, you will be greeted by the Care Coordinator you first spoke with on the phone and they will introduce you to your specialist. After your appointment, your Care Coordinator will discuss the specialists’ recommendations and payment options. Your Care Coordinator will then be your support person and is always available to answer any questions you may have during the process and will schedule any appointments you may need.
